I, the undersigned release One-Way Evangelistic Ministries of Cheyenne, WY or any person associated with One-Way Evangelistic Ministries, whether or not identified in this document, from any and all liability for damages of whatever kind or nature which may at any time result to me, my heirs, or family, on account of compliance or any attempts to comply with this authorization, excepting only the communication of knowingly providing false information.
I further state that I have carefully read this liability release waiver and know the contents thereof and I sign this release as my own free agent and of my own free will.
I fully understand that this is a legally binding release, which I have read and understand. I understand that I may consult with an attorney before signing this document. A facsimile or photocopy of this authorization shall be as valid as the original.
I am aware that as a member (full time, short term, employee, volunteer or other) of OWEM I expose myself to potential hazards which include but are not limited to: car accidents, property damage, falls, illness from contact with others, back injury from lifting, etc. I am voluntarily participating with OWEM with the knowledge of potential hazards and herby agree to accept any and all of these risks. I understand OWEM accepts no financial responsibility in the event of any such hazard.
I agree that my assignees, heirs, distributes, guardians, and other legal representatives will NOT make a claim against, or sue for injury, death or damage resulting from negligence or other acts, howsoever as a result of my ministry with OWEM. I hereby release One-Way Evangelistic Ministries from all actions, claims, or demands that I, my assignees, heirs, guardians and legal representatives now have or may hereafter have for injury resulting from my involvement with OWEM.
If my service with OWEM at any time includes driving an automobile, I acknowledge that I have both a valid driver’s license and an active automobile liability insurance policy as required by state law. I agree to maintain my license and insurance in good standing for my entire tenure as a volunteer for the organization. I am knowledgeable of and agree to abide by local and state traffic laws.
I hereby waive, release and hold harmless from liability One-Way Evangelistic Ministries, its staff, employees, volunteers and agents with regard to any decision that it makes with regard to my request to voluntarily minister with OWEM. I also acknowledge that both OWEM and I have the right to terminate this relationship with or without cause based on the guidelines provided at the commencement of my ministry with OWEM.
